Fault always goes to the leader and glory to the troops. Even when a lower level trooper or staff member screws up it still is fault of the leadership for not instilling better discipline, having a more restrictive selection process, providing oversight, etc, etc, etc...when you finish your book on tactics you should write one about battlefield leadership.
